Diamond Membership Terms and Agreements
DIAMOND MEMBERSHIP PAYMENT:

The Diamond Membership has a 6-month term commitment from the sign-up date.

Your first month's membership dues, of the membership program chosen, are due on the day of sign-up and then subsequently due on the same day of each month ("Monthly Due Date") after that, until canceled at 6 month cycle.

You will be charged monthly on the Monthly Due Date according to your membership program chosen with the preferred payment placed on file.

A valid preferred payment, email, and date of birth are required to participate in LashBar™'s membership program.
DIAMOND MEMBERSHIP TERMS & RIGHT TO CANCEL:

Membership will be automatically renewed after the 6-month term and will only be canceled upon your request.

You have a 15-day grace period to cancel your membership renewal and request a refund for renewed payment.

In the event of cancellation before the 6-month term concludes, a cancellation fee of $150 will be applied.

You can cancel your membership at any time, either in person at the salon or over the phone with the sign-up location, subject to the cancellation fee.

Cancellation of your membership will terminate any benefits including the redemption of the unused membership credits.

LashBar™ reserves the right to modify the terms and conditions of any membership upon written notice.

LashBar™ reserves the right to cancel your membership at any time, subject to the cancellation fee.

Cancellation is restricted to the location where the purchase was made.
ADDITIONAL DIAMOND MEMBERSHIP TERMS:

You may book a service different from your membership type and pay the difference if the service is of higher value. For example, booking a master service with a senior membership.

Unused membership fills roll over to the next month but is non-refundable once the membership is canceled.

All memberships are non-refundable.

Memberships are non-transferable and are valid only for the individual named on the membership.

You may redeem membership credits at any LashBar™ location.
APPOINTMENT CANCELLATION:

Any changes or cancellations to any appointments must be made at least 24 hours prior to service time and date.

Any changes, cancellations, or no-shows made within 24 hours will be subject to LashBar™'s change, cancellation, and no-show policies. Any changes or cancellations to appointments made same day must be made within 1 hour of booking. Any changes, cancellations, or no-shows made after 1 hour will be subject to LashBar™'s change, cancellation, and no-show policies.

You may not use membership credits for applicable LashBar™'s change, cancellation, and no-show policies.
MEMBERSHIP FREEZE :

You may not freeze your membership at any time during your membership.
SALON AND SERVICES:

LashBar™ cannot guarantee that a certain Stylist will be available at such times you are available, nor that there will be availability with a certain stylist level of your chosen program. It is suggested to pre-book your appointments to guarantee service at your requested time.

LashBar™ may be closed from time to time due to holidays, weather, repairs, or any other unexpected events.
EMAIL ADDRESS:

Providing your email address allows LashBar™ to send occasional emails updating you on the membership and package terms, any other news, or marketing.

By purchasing a membership, you agree that LashBar™ may use your email to provide formal notice with respect to the terms, including cancellation and termination of this agreement, and your consent to use your email for this purpose.

By signing our consent form you acknowledge and agree to the terms of service, cancellation policies, procedure process, and any potential risks. By signing this form you also give us permission to use photographs of your service in our social media.
Package Terms and Agreements
PACKAGE TERMS:

Can only be redeemed with the stylist level that was purchased

Can only be redeemed with the service that was purchased

Packages never expire

Non-refundable

You may redeem packages at any LashBar™ location starting Nov 1, 2022

LashBar™ reserves the right to modify the terms and conditions of any membership package upon written notice and to cancel your membership at any time.
APPOINTMENT CANCELLATION:

Any changes or cancellations to any appointments must be made 24 hours prior to service time and date to avoid cancellation/rescheduling fees.

Any changes, cancellations, or no-shows made within 24 hours will be subject to LashBar™'s rescheduling, cancellation, and no-show policies.

Failure to attend an appointment without notice will be charged 100% of the service charge.

Appointments canceled or rescheduled within 24 hours of the service will be charged 50% of the service.

Appointments made same day must be modified or canceled within 1 hour of booking the appointment to avoid cancellation/rescheduling fees.
SALON AND SERVICES:
